- Collaboration between primary care and public health is facilitated when the vision and goals are jointly determined and systematically and continuously communicated across all levels from executive directors to senior managers to front line practitioners;
- Collaboratively developed strategic plans that incorporate a mix of population and individual approaches can work synergistically to address local community health needs;
- Policies and procedures can be very helpful to ensure a common understanding of processes as well as roles of all players in the collaboration;
- Formal written agreements related to sharing confidential client related information need to be established.
