Lessons Learned

• Collaboration between primary care and public health is facilitated by organizational leaders who are champions who are visionary and committed to action that moves the collaborative forward;
• Being a champion can be challenging when resources are scarce;
• The vision and drive of one leader can be highly influential for collaborations;
• A leadership’s presence needs to be ongoing to maintain successful collaborations.
